Mechanical thrombectomy after 16 hours in acute ischemic stroke at Wockhardt Hospital, Nagpur

Mr. DS, was happily attending his son’s marriage in his village about 350 Kms from Nagpur when he developed acute onset of right hemiparesis with inability to speak. He was referred to us but due to the distance he could reach the hospital about 16 hours from the onset. On arrival his symptoms had progressed and he had severe disabling stroke with right hemiplegia, impaired right sided visual field and global aphasia which means complete inability to speak or understand spoken language. He underwent an emergency MRI brain with angiogram which showed occlusion of the left sided blood vessels of brain with patchy infarcts. These imaging findings pointed towards a clear clinicoradiological mismatch which suggested that his brain still had a significant amount of tissue that was not irreversibly damaged and hence could be saved. 

Hence patient was taken for mechanical thrombectomy by Dr. Amit Bhatti- Interventional Neurologist and Stroke Specialist at Wockhardt Hospital, Nagpur and artery was opened using combination aspiration catheter and stent retriever. 

Post procedure patient had a remarkable recovery within 24 hours of procedure. After 3 months of follow up, the patient had recovered completely. 

Mechanical thrombectomy is a novel procedure in which special devices like retrievable stents and aspiration catheters are used to retrieve clot from the vessels in the brain. This is procedure is usually recommended within 6 to 8 hours from the onset of paralysis, however in selected cases the window period  can be extended upto 24 hours. So, if patients experience symptoms of stroke like drooping of face to one side, imbalance, loss of vision, weakness of arms or leg of one side or difficulty in speaking or understanding spoken language then they should immediately rush to a stroke-thrombectomy ready centre which is capable of treating such cases.
